The Effect of Standardized Healthcare processes on Patient Safety and Healthcare Outcomes: A Mixed-Methods Analysis

dc.description.abstractBackground: Patient safety is a global healthcare priority, however, the healthcare system in Palestine continues to encounter structural, resource, and organizational challenges that hinder the standardization of health services. Although accreditation initiatives internationally play a significant role in enhancing patient safety and quality of care, limited evidence exists regarding their effectiveness within Palestinian hospitals. Aim: This study aimed to assess the effects of healthcare standardizations on patient-safety indicators and clinical outcomes in two Palestinian hospitals and to explore healthcare providers’ and patients’ perceptions regarding the implementation of standardization of health services. Methods: An explanatory sequential mixed-methods design was employed. In the first phase, quantitative data were retrospectively extracted from electronic medical records and health records before and after the implementation of standardized clinical protocols at Al-Istishari Arab Hospital and Ibn Sina Hospital. Indicators involved medication errors, Catheter-Associated Urinary Tract Infection, Central Line-Associated Bloodstream Infection, patient falls, 30-day readmissions, length of stay, patient satisfaction, and safety culture perceptions among staff. Quantitative data were analyzed using the Statistical Package for the Social Sciences (SPSS) version 26. In the second phase, we collected qualitative data collected through 19 semi-structured interviews with healthcare providers and 40 discharged patients. We employed thematic analysis to systematically identify key patterns and themes concerning the importance of protocols, feasibility of implementation, perceived impact, and barriers to adoption. Results: Post-accreditation standardization revealed varying effects between the two hospitals. At Al-Istishari Arab Hospital (IAH), medication errors decreased by 17.9%, along with modest improvements in patient satisfaction and safety culture. However, patient fall rates surged by 64.3%, and infection-related indicators, such as CAUTI and CLABSI, showed no statistically significant changes. In contrast, Ibn Sina Hospital (ISH) achieved more significant improvements, with reductions in medication errors (60%), patient falls (58.5%), and length of stay (45.47%), as well as greater enhancements in patient satisfaction. Nevertheless, infection-related indicators and 30-day readmission rates exhibited limited improvement, with no statistically significant changes noted. Overall, infection-related outcomes displayed inconsistent patterns across both hospitals, indicating sensitivity to contextual and operational factors. Qualitative findings underscored the importance of leadership support, communication quality, staff workload, and ongoing training in ensuring adherence to standardized protocols and perceived effectiveness. These results emphasize that sustained time, organizational commitment, and adequate resource allocation are crucial for fully integrating standardization into routine clinical practice. Conclusions: The standardization of healthcare enhances specific patient safety indicators and healthcare outcomes. However, its effectiveness relies on organizational readiness and commitment, leadership involvement, and resource availability. Cultivating a safety culture, providing continuous training, and improving monitoring practices are crucial for maximizing the benefits of standardization and accreditation. en
dc.description.abstractتعدّ سلامة المرضى إحدى أولويات الأنظمة الصحية عالميًا، إلا أن النظام الصحي في فلسطين ما يزال يواجه تحديات هيكلية، ونقصًا في الموارد، ومعوّقات تنظيمية تؤخر تطبيق توحيد الخدمات الصحية. وعلى الرغم من الدور المهم الذي تلعبه مبادرات الاعتماد الصحي عالميًا في تعزيز الممارسات الآمنة، فإن الأدلة المتوفرة حول فعاليتها في المستشفيات الفلسطينية ما تزال محدودة. هدفت هذه الدراسة إلى تقييم أثر تطبيق التوحيد (المعيارية) على مؤشرات سلامة المرضى والنتائج السريرية في مستشفيين فلسطينيين، إضافةً إلى استكشاف تصورات مقدمي الرعاية الصحية والمرضى حول تطبيق توحيد الخدمات الصحية. اُستخدم تصميم البحث المختلط المتقارب؛ حيث جُمعت البيانات الكمية بشكل رجعي من السجلات الطبية الإلكترونية والسجلات الصحية قبل وبعد تطبيق البروتوكولات المعيارية في مستشفى العربي الاستشاري ومستشفى ابن سينا. شملت المؤشرات: الأخطاء الدوائية، التهابات المسالك البولية المرتبطة بالقسطرة، التهابات مجرى الدم المرتبطة بالقسطرة الوريدية المركزية، سقوط المرضى، إعادة الإدخال خلال 30 يومًا، متوسط مدة الإقامة، رضا المرضى، وتصورات العاملين حول ثقافة السلامة. كما جُمعت البيانات النوعية من خلال مقابلات شبه مهيكلة مع مقدمي الرعاية الصحية والمرضى الذين تم خروجهم من المستشفى، وحُلِّلت باستخدام منهجية التحليل الموضوعي لتحديد الأنماط المتعلقة بأهمية البروتوكولات، قابليتها للتطبيق، أثرها المتصوَّر، ومعوّقات التنفيذ. أظهرت النتائج تحسنات متفاوتة بين المستشفيين بعد الاعتماد. ففي مستشفى العربي الاستشاري، انخفضت الأخطاء الدوائية بنسبة 17.9%، مع تحسن محدود في رضا المرضى وثقافة السلامة، إلا أن معدلات سقوط المرضى ارتفعت بنسبة 64.3%، بينما ظلت التغيرات في معدلات التهابات المسالك البولية المرتبطة بالقسطرة التهابات مجرى الدم المرتبطة بالقسطرة الوريدية المركزية محدودة. في المقابل، حقق مستشفى ابن سينا تحسنات أكثر وضوحًا، شملت انخفاضًا في الأخطاء الدوائية بنسبة 60%، وسقوط المرضى بنسبة 58.5%، ومتوسط مدة الإقامة، إلى جانب تحسن أكبر في رضا المرضى، في حين ساءت مؤشرات العدوى في التهابات المسالك البولية المرتبطة بالقسطرة و التهابات مجرى الدم المرتبطة بالقسطرة الوريدية المركزية ومعدلات إعادة الإدخال. وبوجه عام، اتسمت اتجاهات مؤشرات العدوى بعدم الاتساق، مما يشير إلى حساسيتها للعوامل السياقية والتشغيلية. كما أبرزت النتائج النوعية الدور المحوري لدعم القيادة، وجودة التواصل، وعبء العمل، والتدريب المستمر في تعزيز الالتزام بالبروتوكولات المعيارية وتشكيل فعاليتها المتصوَّرة. تخلص الدراسة إلى أن توحيد الخدمات الصحية يُسهم في تحسين سلامة المرضى والنتائج السريرية، إلا أن فعاليته تعتمد على جاهزية المؤسسات، والتزام القيادة، وتوفر الموارد. ويُعدّ تعزيز ثقافة السلامة، والتدريب المستمر، وتحسين آليات المتابعة والرصد عوامل أساسية لضمان ترسيخ آثار التوحيد والاعتماد الصحي واستدامتها في المستشفيات الفلسطينية.ar
